The safety of the children we serve is our number one priority.

In order to qualify as an approved driver for Movin Kidz, you'll need to follow a few simple steps and fill out a couple of our driver certification forms. Don't worry, it's easy and you'll be driving as soon as possible!

Movin Kidz Driver Certification

STEP 1

Certification Packet

Complete Your Movin Kidz Driver Certification Packet. (Click Here)

Fill out and submit our all-in-one, online Driver Certification Packet:

  • Movin Kidz Independent Contractor Agreement form

  • Service Provider Driver Registration form

  • Acknowledgment and Agreement form

  • Background Checks and Drug Test forms*

  • Fingerprint Verification form and forms W-9, 52Z and I-9
    1. Consumer/Investigation Report: Notice, Disclosure, and Authorization to Obtain Consumer/Investigation Reports form,
    2. Consumer/Investigation Reports: Release of Liability form, 3. Backgroundchecks.com (BGC) Required Fields for
    Contract Drivers form, 4. Consent and Direction to Release Drug and Alcohol Testing Information form

STEP 2

Mechanical Inspection

Download & Print Mechanical Inspection Certification Form. (Click Here)

Have your vehicle inspected by an ASE Certified Mechanic and have them complete the printed Mechanical Inspection Certification form (approx. $40).

STEP 3

Driver Certification Files

Upload Your Driver Certification Files. (Click Here)

Provide a scanned copy or picture of all 7 items listed below.

  1. Obtain an MVR from your local DMV (Motor Vehicle Report—driving history)
    You must get your COMPLETE FLORIDA HISTORY—not just a 3-year or 7-year report.

  2. Your Social Security Card

  3. Your valid driver's license

  4. Your vehicle's registration

  5. Proof of insurance

  6. Completed, signed, and approved Mechanical Certification form (from Step 2)

  7. Completed certificate from online PASS exam (Passenger Service and Safety) (from Step 4)

STEP 4

Final Steps

(Some of the companies in the following steps will require a fee for their services.)

  • SCHEDULE a Fingerprint/background check: You’ll be notified of the date and time of your appointment. (approx. $95)

  1. After completing your driver certification packet, a MovinKidz representative will schedule an appointment for you with the Hillsborough County School District and you’ll be notified of the date and time of your appointment.

 

TO FINGERPRINT, YOU MUST BRING THE FOLLOWING:

  1. Completed form 52Z and completed Fingerprint Verification Form (from your driver certification packet provided in Step One. (You can find these forms as an attachment to the email that was sent to you when you completed your driver certification packet.)

  2. A printed copy of e-mail from Movin Kidz representative showing fingerprinting appointment at Hillsborough County School District.

  3. Photo ID

  4. Original Social Security Card

  5. Money order in the amount of $93.00 made payable to HCPS or credit card (a $4.95 transaction fee will be applied to credit card payments).

  • SCHEDULE Drug Test (approx. $50)

  1. Call Test on Time at 310-310-1088 to schedule an appointment (Ask for Julian).

  • Time and location will be provided to you.

  • COMPLETE the online PASS exam (Passenger Service and Safety). (approx. $15)
